What happens next

  1. We contact the patient. Our care coordinator reaches out within one business day to help schedule an appointment.

  2. We confirm the next steps. Our team reviews the service requested, appointment options, and insurance or private-pay arrangements. Coverage depends on the patient’s plan and service.

  3. We provide personalized care. A Registered Dietitian assesses nutrition needs and develops a practical care plan. Additional medical services or labs depend on the program and clinical needs.

  4. We coordinate with your office. With patient authorization, we send visit notes to the referring provider within 1 week of the initial visit, and again whenever the care plan changes or communication is needed.
