  • If you have one of the insurance plans that we are unable to verify, please contact the Member Services (the phone number is on the back of your insurance card) before your appointment and ask the following questions:

    1. Is Medical Nutrition Therapy (nutrition counseling) covered under my plan?

    2. Are the following CPT codes covered?

      • 97802 – Initial Medical Nutrition Therapy Assessment

      • 97803 – Follow-up Medical Nutrition Therapy

      • 97804 – Group Medical Nutrition Therapy

    3. Is a referral or prior authorization required?

    4. Are there any visit limits or frequency restrictions?

    5. What are my copay, coinsurance, deductible, or other out-of-pocket responsibilities?

  • Medicare, Aetna Medicare Advantage, and certain UnitedHealthcare Medicare Advantage plans may only cover Medical Nutrition Therapy for Type 2 Diabetes or Chronic Kidney Disease. A referral from your primary care provider is required and must be faxed to 401-633-6956 at least 3 business days before your appointment.
