Choose your provider, service, location, and appointment time. Please note that in-person appointments may have longer wait times than virtual visits.
After booking, you’ll receive a confirmation email with instructions to:
Complete your Healthie registration
Submit intake forms and insurance information before your visit. If applicable, telehealth access details will be sent prior to your appointment.
Medicare, Aetna Medicare Advantage, and certain UnitedHealthcare Medicare Advantage plans may only cover Medical Nutrition Therapy for diabetes or chronic kidney disease. A referral from your primary care provider is required and must be faxed to 401-633-6956 at least 3 business days before your appointment.
