Scheduling your appointment is easy with our secure online portal through Healthie.
Select your preferred provider, service, and appointment time.
Once prompted, complete the preliminary information needed to book your appointment.
Once booked, you’ll receive a confirmation email with a link to complete your registration in Healthie.
Complete your intake forms and insurance information before your visit.
You will be notified of any additional details, including telehealth access instructions (if applicable) prior to your appointment.
Medicare, Aetna Medicare Advantage, and certain UnitedHealthcare Medicare Advantage plans limit Medical Nutrition Therapy coverage to qualifying diagnoses of diabetes or chronic kidney disease. Patients with these plans must also have a referral from their primary care provider on file before seeing a Registered Dietitian Nutritionist. Please ask your provider to fax the referral to 401-633-6956 at least 3 business days prior to your appointment.
