Our Metabolic Weight Management (MWM) Program is a 12-week Medical Nutrition Therapy program featuring 6 personalized one-on-one MNT sessions alternating with 6 group MNT sessions—designed to provide individualized care, accountability, and community support while supporting referring providers through coordinated, outcomes-driven care.
The MWM Program is a science-based approach to achieving sustainable weight loss and improving overall metabolic health. Through personalized nutrition plans, physical activity guidance, stress management support, and evidence-based behavior change strategies, we help you optimize your metabolism and build lifelong healthy habits. Led by experienced registered dietitians and health coaches, you’ll receive expert guidance, accountability, and education every step of the way.

INITIAL & FINAL COMPREHENSIVE ASSESSMENTS
Your journey starts and ends with a comprehensive metabolic and lifestyle assessment with your registered dietitian. This includes:
Detailed nutrition and lifestyle evaluation
Review of recent labs (A1C, fasting glucose, lipids, triglycerides, insulin*)
Body composition analysis
Medical history review
Current habits, stress, sleep, and environment assessment
Cardiometabolic risk screening
(*Lab work ordered by your physician or NP; we coordinate closely with your medical team.)

PHASE 1: CORE 12-WEEK PROGRAM (1:1 + GROUP SESSIONS)
This is the heart of the program. A structured 12-week metabolic transformation that combines personalized nutrition therapy with group-based education and support.

You will meet bi-weekly with your registered dietitian for individualized guidance, nutrition planning, lifestyle coaching, accountability, and weekly goal setting.
✔ 6 X Live, Virtual Group MNT Sessions
Alongside your individual visits, you will participate in bi-weekly virtual group sessions led by our registered dietitians and health coaches.
These sessions cover:
Introduction and goal setting
Hydration and metabolism
Balanced eating
Healthy digestion
Physical activity for metabolic health
Meal prep and planning + Next steps
The combination of weekly 1:1 visits + weekly group MNT sessions is the gold standard, evidence-based model for achieving the best measurable outcomes for patients. Our practice provides education, community, and the consistency people need to achieve cardiometabolic health.

PHASE 2: LIFESTYLE INTEGRATION PROGRAM (1:1 + GROUP SESSIONS)
The Lifestyle Integration Program is a 12-week continuation phase that supports long-term metabolic health through ongoing nutrition education, lifestyle reinforcement, and group-based accountability. This phase is designed to help participants maintain clinical improvements, strengthen self-management skills, and integrate sustainable nutrition and lifestyle practices into daily life

PHASE 3: CONTINUED PERSONALIZED SUPPORT
Because lasting change can often take 6 months to a year or more, many clients choose ongoing support after Phase I & II. Continued care may include:

Updated lab reviews
