Why is Cardiometabolic Health Essential for Life?

Our Metabolic Weight Management Program goes far beyond traditional weight loss. We help you strengthen your metabolic health and restore your cardiometabolic health — the overall well-being of your cardiovascular and metabolic systems, including blood pressure, cholesterol, blood sugar, triglycerides, and abdominal fat.

Cardiometabolic health is defined by the absence of risk factors for cardiovascular disease and type 2 diabetes. When these systems are functioning well, you reduce your risk for heart attack, stroke, insulin resistance, and chronic inflammation.

Our goal is to help you improve these internal markers, reduce disease risk, and create sustainable habits that support long-term metabolic resilience. And if weight loss is also your goal — the Metabolic Weight Management Program provides the blueprint for weight loss in a way that feels doable, supportive, and grounded in evidence-based care.

Why Cardiometabolic Health Matters?

Many people struggle with metabolic and weight-related concerns such as:

  • High blood pressure

  • Elevated triglycerides or LDL cholesterol

  • High fasting blood sugar or A1C

  • Excess abdominal fat

  • Insulin resistance

  • Chronic stress and poor sleep

  • Weight loss resistance

These are early signs of cardiometabolic dysfunction—but with the right lifestyle, nutrition, and behavior-change support, they are also highly reversible.

Our program is designed to identify these risk factors early and help you reverse, reduce, or stabilize them.

Is This Program Right for You?

This program is a strong fit if you’re experiencing:

  • Weight gain or difficulty losing weight

  • Prediabetes, insulin resistance, or elevated blood sugar

  • High cholesterol or triglycerides

  • High blood pressure

  • Elevated waist circumference

  • Fatigue, inflammation, or poor metabolic flexibility

  • A desire to prevent type 2 diabetes or heart disease

  • A history of yo-yo dieting or inconsistent habits

  • The desire to feel healthier, stronger, and more confident

If this sounds like you—you are not alone, and you are in the right place.

Your Personalized Metabolic Transformation Journey

Initial and Final Comprehensive Baseline Assessments (1:1 Sessions at Week 1 and Week 12)

Your journey starts and ends with a comprehensive metabolic and lifestyle assessment with your registered dietitian. This includes:

  • Detailed nutrition and lifestyle evaluation

  • Review of recent labs (A1C, fasting glucose, lipids, triglycerides, insulin*)

  • Body composition analysis

  • Medical history review

  • Current habits, stress, sleep, and environment assessment

  • Cardiometabolic risk screening

(*Lab work ordered by your physician or NP; we coordinate closely with your medical team.)

We use this information to shape a personalized plan that targets metabolic and cardiometabolic improvement—not just weight loss.

Phase 1: Core 12-Week Program (1:1 + Group Sessions)

This is the heart of the program — a structured 12-week metabolic transformation that combines personalized nutrition therapy with group-based education and support.

✔ Twelve 1:1 Medical Nutrition Therapy (MNT) Sessions

You will meet weekly with your registered dietitian for individualized guidance, nutrition planning, lifestyle coaching, accountability, and weekly goal setting.

✔ Twelve Weeks of Live, Virtual Group MNT Sessions

Alongside your individual visits, you will participate in a weekly virtual group session led by our registered dietitians and health coaches.
These sessions cover:

  1. Introduction and goal setting

  2. Hydration and metabolism

  3. Balanced eating

  4. Healthy digestion

  5. Physical activity for metabolic health

  6. Meal prep and planning

  7. Emotional eating

  8. Sleep and stress management

  9. Mindfulness and your health

  10. Healthy habits for life

  11. Overcoming challenges

  12. Wrap-up and next steps

The combination of weekly 1:1 visits + weekly group MNT sessions is the gold standard, evidence-based model for achieving the best measurable outcomes for patients. Our practice provides education, community, and the consistency people need to achieve cardiometabolic health.

✔ Continuous Monitoring & Support

Throughout the 12 weeks, we track:

  • Weight and body composition

  • Blood glucose (if applicable)

  • Blood pressure (if applicable)

  • Energy, sleep, stress, and eating patterns

  • Weekly progress toward goals

This phase is where patients typically see significant improvements in cardiometabolic markers and overall wellbeing.

Phase 2: Continued Personalized Metabolic Health Planning and Support

Because lasting change doesn’t happen overnight, many clients choose ongoing support after the initial 12 weeks. Continued care may include:

  • Monthly or bi-weekly follow-up visits

  • Ongoing accountability

  • Adjustments to nutrition, fitness, or stress routines

  • Updated lab reviews

  • Continued focus on cardiometabolic health markers

This phase ensures long-term success and stability.
