The Metabolic Weight Management Program is a science-backed, high-touch approach to achieving sustainable weight loss and better metabolic health. This hybrid program blends personalized one-on-one sessions with a registered dietitian and dynamic group coaching every other week, giving you the best of both worlds: expert guidance tailored to your unique needs and the motivation and accountability that comes from a supportive community.

Over 13 weeks, you’ll focus on the core pillars of metabolic health—hydration, balanced eating, physical activity, mindful eating, stress management, and quality sleep. Your progress is continuously tracked and adjusted to help you not only reach your goals but maintain them long-term.

What’s Included:

Comprehensive Wellness Assessments Start strong and finish stronger. At the beginning and end of your program, we’ll complete an in-depth wellness assessment—including weight, body composition, nutrition habits, and lifestyle behaviors. These insights help us track your progress and fine-tune your personalized plan for the best possible results.

Personalized 1:1 Nutrition Sessions Work one-on-one with a registered dietitian to create a tailored nutrition and lifestyle plan designed specifically for your health goals and medical needs. Each session provides expert guidance, practical strategies, and encouragement to help you make lasting changes.

Group Support for Motivation & Accountability Every other week, join our group classes for community support, expert coaching, and added accountability. These sessions are designed to keep you focused, motivated, and inspired as you build healthy habits alongside others on the same journey.

Continued Care Beyond 13 Weeks Your progress doesn’t stop at the end of the program. After 13 weeks, you’ll meet with your dietitian to evaluate your results and create a Phase 2 care plan. This might include continuing with the hybrid schedule, shifting to only 1:1 or group sessions, or meeting less frequently—whatever helps you maintain and build on your success.

Coordinated Care with Your Physician With your permission, we’ll share progress reports with your physician, outlining your health improvements and any plan updates. This ensures a collaborative, whole-health approach to your care.

1. What is the Metabolic Weight Management Program?

Our program is a structured, medically supervised nutrition and lifestyle program designed to support sustainable weight management. It is led by registered dietitians and focuses on medical nutrition therapy (MNT) with evidence-based strategies for metabolic health, weight loss, and long-term wellness.

2. How does insurance billing work for this program?

We will bill your insurance for 1:1 and group MNT sessions whenever coverage is available. Most major insurance plans cover MNT. Before starting, our team will verify your benefits and confirm coverage for 1:1 and the group MNT program.

3. Do I need a referral from my primary care physician (or other healthcare provider) to participate in this program?

Rarely, an insurance plan may require a referral from your primary care physician for medical nutrition therapy. If your plan requires a referral, we will guide you through the process to make sure all documentation is in place before your first session.

4. Why do you need my health insurance information?

We need your health insurance information so we can verify your coverage and bill your insurance company for your 1:1 and group program sessions.

5. Why do you need my credit or debit card information if I decide to join the Metabolic Weight Management Program?

We require a credit or debit card on file to cover any fees for missed 1:1 appointments or late cancellations (per our 48-hour cancellation policy). This ensures a smooth process and avoids interruptions in your care.

6. What is the cancellation policy?

We have a 48-hour cancellation policy for all 1:1 appointments. If you cancel or reschedule within less than 48 hours of your appointment time, or if you miss your appointment without notice, a fee will apply. This ensures that appointment slots remain available for other patients in need.

7. Are there any rules or guidelines for this program?

Yes. To get the most out of this program, we ask that participants:

  • Attend all scheduled sessions.

  • Actively participate and complete session assignments.

  • Use Healthie, our secure EHR portal, to access program materials, communicate with your dietitian, and track progress.

    These guidelines help you stay accountable and ensure program effectiveness.

8. I already have a wellness account with the city. Do I have to create another account in Healthie, the Electronic Health Record (EHR) portal if I join The Metabolic Weight Management Program?

Yes. Healthie is our secure electronic health record (EHR) system that houses your nutrition records, appointments, and program materials. Even if you have a city wellness account, you will need to create a Healthie account to participate in the program.

9. Will there be additional fees with this program?

No. There are no additional costs to this program as our program is covered by your health insurance. However, late cancellations for 1:1 or missed 1:1 appointments will incur fees per our late cancellation policy.

10. I am seeing a dietitian at another practice. Can I still join this program?

Yes. You can still join our program even if you are seeing a dietitian elsewhere. However, insurance may not cover duplicate services if you are receiving MNT at two practices at the same time. We recommend checking with your insurance provider before enrolling.

11. I am already seeing a dietitian at your practice. Can I still join this program?

Yes. If you are currently a patient in our practice, you may still enroll in the program. We will coordinate your care so there is no overlap or duplication of services.

12. I am already participating in a group program at your practice. Can I join this one too?

Yes. You may participate in both a group program and The Metabolic Weight Management Program. We will ensure your sessions are scheduled appropriately and billed correctly through your insurance.

13. What information do you need from me to participate in the Information Session?

The Information Session is an overview of the program and does not require detailed medical information. We may ask for your name, contact information, and health insurance details so we can prepare for any follow-up questions you may have about coverage.

14. What information do you need from me if I decide to join The Metabolic Weight Management Program?

To enroll, we will need:

  • Your demographic and contact information

  • Your health insurance information

  • A credit or debit card on file

  • Your health history and any required referrals (if applicable)

  • A signed consent acknowledging our policies (including the 48-hour late cancellation policy)

15. Will you share any personal information (health or other) about me with my boss, colleagues, or others in the group?

No. All of your health information is protected under HIPAA and will not be shared with your employer, colleagues, or other participants. Your participation is completely confidential.

16. Can you tell me if one of my colleagues signed up for the program?

No. For privacy reasons, we cannot disclose who has or has not joined the program. Your colleagues’ participation, like yours, is confidential. You may see one of your colleagues on the group Zoom call. We will review the guidelines on confidentiality for group participants at the Information Session and at the beginning of the Metabolic Weight Management Program.

17. How long is the program?

The core program is designed for 13 consecutive weeks, including an initial comprehensive nutrition assessment and weekly follow-up sessions. This allows us to provide ongoing guidance, accountability, and adjustments tailored to your progress.

18. What can I expect in the sessions?

Each 1:1 session is personalized to your needs and may include:

  • Review of your health history, goals, and progress

  • Nutrition and lifestyle recommendations tailored to your metabolic health

  • Meal planning support and strategies for long-term behavior change

19. Who is this program for?

The program is designed for adults who want to improve their metabolic health, lose weight, and create sustainable lifestyle changes. It is especially beneficial for individuals with conditions such as prediabetes, diabetes, high blood pressure, high cholesterol, or other weight-related health concerns.

20. Can I continue beyond the 13 weeks?

Yes. Many patients choose to continue with ongoing MNT sessions after the initial program to maintain accountability and long-term results. Continued care will typically be covered by most health insurance plans.
