A 24-Week Clinical Reset for Metabolism, Hormones & Life!

Our Medical Metabolic Health Program is a 24-week, high-touch wellness experience designed for individuals ages 15 and up who are ready to take control of their metabolism, hormones, and long-term health. Guided by a team of Registered Dietitians and a Nurse Practitioner, this comprehensive program combines advanced lab testing, wearable tracking tools (like CGMs and Oura Rings), and personalized nutrition and lifestyle protocols to uncover the root causes of stubborn weight, insulin resistance, PCOS, chronic fatigue, and inflammation.

Our integrative care model supports both teens and adults, offering medically supervised options including GLP-1 medications, weight loss prescriptions, and hormone therapy where appropriate. Whether you're struggling with early metabolic dysfunction or seeking a structured, science-backed path to sustainable wellness, this program provides clarity, accountability, and the expert support you need to test, track, and transform.

A Thoughtful Approach for Teens (Ages 15–17)

We welcome individuals ages 15 and older, and take special care when working with teens. Every adolescent participant is screened thoroughly and supported in partnership with their parent(s), guardian, and care team. Our approach prioritizes long-term health, emotional well-being, and evidence-based guidance that empowers—not pressures—young clients.

Our Metabolic Weight Management Program

Who We Serve

This program is ideal for those who:

  • Struggle with stubborn weight, insulin resistance, or PCOS

  • Are tapering off—or considering—GLP-1 medications (e.g., Ozempic®, Wegovy®)

  • Experience fatigue, mood swings, or unexplained inflammation

  • Suspect hormonal shifts (adolescence, perimenopause, menopause, low testosterone)

  • Want a proactive, concierge-level roadmap instead of one-size-fits-all advice

What’s Included:

  • Comprehensive Initial Assessment
Your program begins with a detailed medical and lifestyle intake. This includes a full review of your health history, labs, medications, lifestyle habits, weight trends, and goals. Our team uses this data to create your individualized plan.

  • Weekly One-on-One Visits
You’ll meet weekly with a registered dietitian or nurse practitioner for 12 weeks, alternating between the two based on your care needs. These sessions provide clinical oversight, medication management (if applicable), and personalized nutrition and lifestyle counseling.

  • Group Medical Nutrition Sessions
In addition to your 1:1 care, you’ll have access to small group visits (6–8 participants) led by our NP and RD team. These sessions offer a supportive space to dive deeper into topics like blood sugar balance, hormone health, emotional eating, and sustainable lifestyle change—all while learning from others on a similar path. Many clients find these groups incredibly motivating and reassuring.

  • Customized Medication & Supplement Plan
If appropriate, your nurse practitioner may prescribe GLP-1s, weight loss medications, or bio-identical hormones. Supplement recommendations are also provided based on lab data and your specific needs to support metabolic health.

  • Functional & Conventional Lab Testing
We use a blend of traditional labs and targeted functional testing to identify root causes of metabolic challenges. Labs are reviewed with you and used to adjust your care plan for the best possible outcomes.

  • Ongoing Lifestyle Support
You’ll receive tools, education, and strategies for sustainable behavior change, including meal planning, stress management, movement, sleep optimization, and mindset work. Our approach is realistic, evidence-based, and tailored to your stage of life.

  • Optional Add-Ons & Continued Care
After your initial 12-week intensive, you can continue with follow-up visits, group sessions, or additional support services. Many patients choose to continue long-term to sustain their progress and explore deeper hormone or weight optimization.

What You Can Expect:

While every journey is unique, most clients experience powerful, measurable improvements over the course of this 24-week program. Lab markers like A1c, fasting insulin, and inflammation levels often improve significantly—many clients see their A1c drop from pre-diabetic to normal range, insulin levels cut in half, and visceral fat decrease by 10–25%. But beyond the numbers, the real transformation happens in daily life.

Clients report more consistent energy, fewer cravings, improved sleep, better mood stability, and feeling “clearer” and more in control of their health. For teens and adults alike, it’s about finally understanding what your body needs and seeing those changes reflected in how you feel—physically, mentally, and emotionally. Whether your goal is to come off medications, improve labs, or simply feel like yourself again, this program is designed to help you get there—with support at every step.

Frequently Asked Questions (FAQs)

How is this program different from other weight loss or nutrition plans?

This isn’t another cookie-cutter diet or generic meal plan. It’s a high-touch, medically supervised lifestyle medicine program designed to uncover and address the why behind your symptoms. You’ll work closely with a nurse practitioner and registered dietitians to create a plan that’s built around you.

We use advanced labs, wearable tools, and personalized monitoring to track your progress—not to overwhelm you, but to give you real-time insight into what’s working and what needs adjusting. This data helps us fine-tune your care, so you see meaningful, measurable results—whether that’s lower blood sugar, better sleep, reduced inflammation, or renewed energy.

Is this program covered by insurance?

This is a private-pay program, which allows us to offer more time, deeper testing, and flexible support without the limitations of insurance. Some lab work or prescriptions may be eligible for HSA/FSA reimbursement. We’ll provide you with a superbill if you'd like to submit for possible out-of-network reimbursement.

Is anything in the program eligible for insurance reimbursement?

This is a concierge, private-pay program that offers a level of care beyond what traditional insurance typically covers. However, certain components may be eligible for insurance reimbursement, depending on your benefits:

  • 1:1 Medical Nutrition Therapy (MNT) visits with a Registered Dietitian may be covered by your health plan.

  • Standard lab work (e.g., metabolic panels, lipid testing, thyroid, glucose, A1c) is often reimbursable through your insurance or ordered through in-network lab providers.

We’ll help guide you through what might be reimbursed and provide superbills or insurance-compatible lab orders where appropriate. Our model is intentionally structured outside of insurance to give you more flexibility, faster access, and a fully personalized experience—without the limitations or delays of traditional care.

Who is this program for?

We work with clients 15 years and older who are struggling with insulin resistance, weight gain, fatigue, hormone imbalances, or post-GLP-1 tapering. Whether you’re navigating PCOS, prediabetes, or perimenopause—or just want to optimize your health from the inside out—this program is built for you.

Do you prescribe medications like Ozempic or GLP-1s?

Yes. Our nurse practitioner can prescribe GLP-1 medications (such as Ozempic®, Wegovy®, or compounded semaglutide) as well as other weight-loss prescriptions and bio-identical hormone therapy—when clinically appropriate.

Can teens join this program?

Yes. We accept clients age 15 and older. For teens, we conduct a thorough screening and involve parents or guardians as part of the support team. The focus is on energy, confidence, long-term health, and building a positive relationship with food—not just weight.

How long is the program and what does it include?

The program runs for 24 weeks, with weekly visits (either 1:1 or group), lab testing, personalized nutrition and supplement planning, medication management (if applicable), and high-touch lifestyle support. You’ll also receive access to wearable tools like CGMs and Oura Rings, depending on your package level.

What happens after the 24 weeks?

You’ll receive a personalized long-term plan and the option to continue with ongoing care, quarterly check-ins, additional testing, or group sessions. Many of our clients choose to stay on for continued optimization and support.

Is this program offered virtually or in person?

We offer both! You can choose full virtual access or a hybrid model (registered dietitians only) that includes in-person visits at one of our Rhode Island offices (if you're local). Group sessions and data reviews can be done from anywhere.

The 24-Week Care Framework

A concierge-style medical wellness experience designed to transform your metabolism, hormones, and long-term health—inside and out.

This high-touch, private pay program is guided by a collaborative care team: a Nurse Practitioner (NP) and a Registered Dietitian (RD). You’ll also gain access to small group medical nutrition sessions, wearable tech insights, and advanced diagnostics—all included in your care.

Phase 1: Foundations (Weeks 1–12)

Assess. Understand. Activate.

  • Week 1: Initial Nutrition Consultation (60–90 min)
Start with your RD for a comprehensive nutrition and lifestyle assessment, including symptom mapping, history review, lab interpretation (if available), and personalized goal setting. This session lays the foundation for your individualized plan.

  • Week 2: Medical Visit with Nurse Practitioner
A full medical intake to review your clinical history, current medications, symptoms, and body composition. The NP will:

    • Order your baseline labs (metabolic, hormone, GI, and inflammation panels)

    • Review wearable tech onboarding (e.g., CGM, Oura Ring)

    • Prescribe medications (e.g., GLP-1s, weight loss meds, or hormone therapy) if clinically appropriate

    • Begin medication management, which will be ongoing throughout the program

  • Weeks 3–12: Weekly Rotation of Personalized Support

    • 1:1 appointments with your RD and NP for nutrition therapy, supplement guidance, and medication optimization

    • Small group sessions (6–8 participants) led by the NP + RD team, covering topics like metabolic flexibility, stress & cortisol, cravings, blood sugar balance, and hormone health

    • Real-time review of labs, CGM & Oura data, and progress tracking

    • Behavior and mindset coaching to build early momentum and confidence

Phase 2: Optimization (Weeks 13–24)

Refine. Sustain. Transform.

  • Continued Weekly Rhythm:
The cadence of 1:1 care and group support continues, with a shift toward habit reinforcement, accountability, and lasting change.

  • NP Focus:
Ongoing medication management, follow-up lab testing, and refinement of prescriptions and protocols based on your progress and goals

  • RD Focus:
Behavior layering, gut-hormone balance, meal design, metabolism support, and lifestyle shifts tailored to your evolving needs

  • Week 24: Final Assessment & Long-Term Plan
A comprehensive wrap-up visit with your care team to review clinical improvements, lab changes, and wearable trends. You’ll leave with a personalized maintenance roadmap—and the option to continue with ongoing optimization, quarterly visits, or group care.
